Visitor Request to Participate in the Lord's Supper

An elder will provide a visitor's card to those who desire to participate with us in the sacrament.  The front of the card contains the following:

Welcome to our worship service!  We administer the Lord’s Supper every Sunday. We believe this sacrament is a gift of Christ to His church, a means of grace to feed and nourish our faith (1 Cor. 10:16-17). It is a holy gift, and is to be received in a holy and accountable manner (1 Cor. 11:17-32).  Christ has appointed elders in His church to give oversight to the ministry of Word and Sacraments.  If you would like to participate with us this morning, we ask that you sincerely affirm the following:


  1. You have been baptized with water in God’s Triune Name and have publicly professed your faith as summarized in the Apostles’ Creed (p. 851,Trinity Psalter Hymnal).

  2. You are a repentant believer and trust in Jesus Christ alone for your salvation.

  3. You desire to live a godly life out of gratitude for God’s grace.

  4. You are an accountable member in good standing who regularly attends a Protestant church which:

    • Purely preaches the Gospel

    • Rightly administers the sacraments of Baptism and the Lord’s Supper

    • Faithfully exercises church discipline

  5. You believe the Lord’s Supper is a means of grace given for your spiritual benefit (and not merely a memorial remembrance).

The back of the card contains the following:

YES, I do affirm the five statements above, and request to participate with Covenant URC in the Lord’s Supper. Please provide completed card to an elder or deacon before the service or during the collection.
